265.892           Medical Home Model:

MSSNY will study the medical home model through an existing committee or by establishing a Task Force on Medical Home Models with these directives:

  1. define medical home, including payment models, after considering American Medical Association Policy H-160.919;
  2. research successful medical home pilot projects;
  3. monitor the progress of medical home pilot projects in New York State;
  4. make policy and legislative agenda recommendations on this subject to the MSSNY Council; and
  5. develop a program to educate physicians in New York State about the opportunities and threats inherent in medical home pilot projects. (HOD 2009-94; Reaffirmed HOD 2019)
